Realistic adjunct management of AKI and CKD centres on interrupting a shared injury-maladaptive repair-fibrosis cascade. AKI is driven by proximal tubular mitochondrial dysfunction (loss of PGC-1α, impaired fatty-acid oxidation, NAD⁺ depletion), ROS overproduction, ferroptosis (GPX4/GSH depletion, lipid peroxidation) and necroptosis (RIPK1/3-MLKL). Therefore, Glomefix represents a blend of specific fraction of edible vegetable with components activating Nrf2/HO-1, protecting mitochondria (SIRT1/3, AMPK, PGC-1α), inhibiting ferroptosis/necroptosis and exhibiting anti-inflammation (DAMPs/TLR2/4/NF-κB, NLRP3-IL-1β, TNF-α, IL-6, MCP-1/CCR2). The central target set for slowing and ultimately reversing CKD is modulation of TGF-β/SMAD3. RAAS overactivation in CKD and glomerular hyperfiltration and podocyte injury are advisedly managed with ACE inhibitors/ARBs. Mineral-bone disturbance may benefit from vitamin D3 supplementation. Once Glomefix is used alongside standard care for blood pressure and glucose control, the treatment therefore limits AKI-to-CKD transition, slows progression and ultimately restores lost kidney function.
